Your first appointment:
The Berkeley health department is requiring the following:
The entire office is cleaned, ventilated and disinfected in-between all appointments.
- If possible we ask that new patients fill out the initial intake forms (see below) at home to bring with them on their first appointment, I am happy to mail you the forms if you do not have a printer. If that is not convenient you are very welcome to fill them out here at our office (approx. a 15 min. process).
The Berkeley health department is requiring the following:
- A temperature check when entering the office.
- Please reschedule if if you have a cough, a sore throat, or a fever.
- Please wash your hands before the visit and use office provided hand sanitizer before and after the visit.
- Please arrive on time, if you are early please wait in your car until your appointment time.
- Please wear a mask the entire time you are in the office. We can provide a mask for you if you have forgotten yours.
- We will socially distance 6' in the office except for the acupuncture procedure itself.
